Health and Social Care Team Update:

Over the past few weeks the Health and Social Care Team have been reviewing all our case files, updating our records and, where appropriate, contacting service users to offer an assessment of their needs. If you have not received a letter from us, then call the Health and Social Care Team on 0161 882 2200 and make an appointment.

 

We are using this review period to update our records and we need you to confirm your contact details as soon as possible. We often need to contact you in order to keep you up to date with our services, training events and other activities, which is why it is important that your details are current and correct.

 

Why are we offering an assessment? All new service users are offered an assessment upon joining the agency – where practical we will arrange this immediately, but it will be done with 14 days of first contact with the agency. Existing service users who may not have had an assessment for some time will be offered an assessment date.

 

The assessment process is an important one which allows you, the service user, to talk about your concerns – relationships, family, benefits, immigration, training, employment, or other areas of challenge which may impact upon you. The assessment process allows us, BPNW, to look at the best support package that we can offer you.

 

As an agency we are committed to respecting the confidentiality of any and all information gathered during this assessment process and as an agency have strict guidelines and policies covering confidentiality.
