We are trying to evaluate your needs in terms of HIV peer and social support in Cumbria and Lancashire.

Body Positive North West has kindly offered to distribute this questionnaire for us, so we do not currently hold any information about you.

We would greatly appreciate your views on what support you are currently receiving, if any, or on what type of support you would like in the future.

In order to do this we would like you to complete the attached questionnaire. Please give as much detail as you are able to offer.

The questionnaire is totally confidential, however if you would like feedback on current or future service provision, please leave your preferred method of contact at the end of the questionnaire. Any personal information given will be treated confidentially.
